Most content consists of specimen collecting information. Lists begin with specimen number (K-1 to K-180), locality (including county, state, and landmarks), description of habitat (e.g. terrain or water visibility), collector names (E. E. Ford and P. C. Holt), and notes on Branchiobdellidae abundance. There are notations including additional specimen numbers (1050 to 1232) that were added later. Locations include but are not limited to Tennessee, Arkansas, Oklahoma, Oregon, Washington, Minnesota, Wisconsin, Michigan, Pennsylvania, and Ohio.
